Region 12 Tournament Specific Rules
Emergency Care Form
We highly recommend that each student and adult working with the teams complete an Emergency Care form at the first team meeting. If privacy is a concern, the coach can collect these forms in sealed envelopes with the team members name on front for use only if required.
